Qualifications

Responsibilities

• Provide psychosocial assessments and consultations for patients and families, including Social Drivers of Health and protective factors.
• Collaborate with RN Case Manager/Transition Planner and interdisciplinary team to ensure safe and timely discharge.
• Maintain documentation of assessments, interventions, discharge plans, and barriers in EMR per hospital policy.
• Provide timely social work consultations and coordinate referrals to address social needs and discharge planning.
