Job summary

Kindred Hospital Houston Medical Center and Acute Rehabilitation Unit (ARU) seek a dedicated care coordinator to facilitate patient flow and ensure efficient care delivery across the continuum. You will collaborate with the ICT team, physicians, and families to optimize resources and discharge planning.

The role emphasizes adherence to reimbursement guidelines, care planning, and communication with diverse stakeholders to support best patient outcomes in a fast-paced LTACH/ARU setting.

Qualifications

• Must read, write and speak fluent English.

Responsibilities

• Coordinate clinical and psycho-social activities with the Interdisciplinary Team and Physicians.
• Monitor all areas of patients' stay for effective care coordination and efficient care facilitation.
• Remain current on reimbursement modalities, community resources, case management, psychosocial and legal issues affecting patients and providers.
• Refer high-risk patients who would benefit from additional support.
• Serve as a patient advocate and collaborate to maximize patient and family informed decisions.
• Demonstrate knowledge of growth and development to provide age-appropriate care.
• Participate in interdisciplinary rounds/conferences to review goals and plans of care.
• Coordinate with interdisciplinary care team, physicians, patients, families, post-acute providers, payors in planning care.
