Job summary

Kindred Hospital Denver and Acute Rehabilitation Unit (ARU) seeks a skilled care coordinator to manage patient care across the continuum, collaborating with ICT members and physicians to optimize outcomes and resource use.

The role emphasizes case management, utilization review and discharge planning, with emphasis on patient advocacy and effective communication with families and payors. English fluency is required.

Qualifications

• Postsecondary education from an accredited school of nursing or social work.
• BSN/MSN/BSW/MSW preferred.
• Must read, write and speak fluent English.

Responsibilities

• Coordinate clinical and psychosocial activities with the Interdisciplinary Team and Physicians.
• Monitor patient stays for effective care coordination and discharge planning.
• Collaborate with payors, providers and families to plan care throughout the continuum.
• Support care transitions and ensure appropriate resource use and patient advocacy.
