Job summary

Family Health Centers of San Diego is seeking a full-time social services professional to support high-need, medically underserved clients across multiple sites in San Diego. You will assess needs, develop formal care plans, coordinate services, and provide therapeutic interventions to improve health outcomes.

Requirements include a Bachelor’s degree in a related field, 3 years' experience, and a valid California driver’s license; a Master’s degree is preferred.

Qualifications

• 3 years of work experience providing human services to high risk, medically underserved populations.
• Bachelor’s degree in a related field required; Master’s degree preferred.
• Valid CA driver’s license and willingness to travel between sites.

Responsibilities

• Completes documentation accurately and timely per department standards.
• Assesses client health and support service needs and creates formal care plans.
• Delivers therapeutic interventions and provides intensive client support.
