Job summary

Family Health Centers of San Diego (FHCSD) is hiring a Case Manager II to provide intensive, therapeutic case management to medically underserved individuals. This grant-funded role involves developing care plans, conducting assessments, and coordinating services across multiple sites in San Diego County.

The ideal candidate has a relevant Bachelor's degree (Master's preferred), 3 years of experience, and strong counseling and communication skills.

Qualifications

• 3 years of work experience providing human services to high risk, medically underserved populations.
• Bachelor's degree in Social Science, Public Health, Healthcare Administration, or related field.
• Master's degree preferred.
• Willingness to travel between sites and locations as required.
• Valid California driver's license with insurance; acceptable driving record.

Responsibilities

• Complete documentation accurately and timely per department standards.
• Conduct initial and ongoing assessments of clients' health and support needs.
• Develop formalized care plans with measurable goals for high-need clients.
• Provide intensive individual support and counseling as needed.
• Coordinate internal and community referrals and services.
