Job summary

Family Health Centers of San Diego is seeking a Case Manager II to deliver intensive community-based care to medically underserved individuals in the Lemon Grove area. The role emphasizes therapeutic interventions, comprehensive care planning, and collaboration across sites.

The ideal candidate has at least 3 years of relevant experience, a Bachelor's in a related field, and a preference for a Master’s degree. A valid California driver’s license and ability to travel between sites are required.

Qualifications

• 3 years of work experience providing human services to high risk, medically underserved populations.
• Bachelor's degree in Social Science, Public Health, Healthcare Administration, or closely related field.
• Master's degree preferred; equivalent education and experience acceptable.

Responsibilities

• Completes all required documentation accurately and timely.
• Assists in preparing reports as required.
• Conducts initial and ongoing assessments of clients' health and support needs.
• Sets level of client need and creates formal case plans with measurable goals.
• Develops and updates written care plans with clients.
• Provides therapeutic interventions and intensive individual support.
• Focuses on medical adherence and appointment coordination.
• Performs other duties as assigned.
