Job summary

Family Health Centers of San Diego (FHCSD) is seeking a Case Manager II to deliver intensive case management and therapeutic interventions across our network. The role emphasizes working with medically underserved populations and coordinating community-based health services to improve adherence and outcomes.

The position requires at least 3 years of related experience, a relevant bachelor's degree (master's preferred), and a valid California driver’s license for occasional site travel.

Qualifications

• 3 years of work experience providing human services to high-risk, medically underserved populations.
• Bachelor's degree in Social Science field, Public Health, Healthcare Administration, or closely related field.
• Master's degree in relevant field preferred.
• Or equivalent combination of education and experience that provides the skills needed.

Responsibilities

• Completes all required documentation accurately and on time.
• Conducts initial and ongoing assessment of client health and support needs.
• Creates formalized case plans and goals with high-need clients with measurable outcomes.
• Provides therapeutic interventions and intensive client support, including referrals.
• Performs other duties as assigned.
