Qualifications

• 3+ years’ experience with homeless population.
• Ability to work with a culturally diverse population.

Responsibilities

• Under the supervision of the Program Supervisor, ensure quality service delivery to individuals and families in need of homeless prevention services.
• Provide supervision to case managers and oversee intensive case management for program participants and their families.
• Arrange, coordinate, monitor and deliver services related to meeting housing needs.
• Assure timely intakes, assessments, service plans and referrals.
• Participate in development of After-Care and ongoing housing assistance service planning processes.
• Coordinate supportive services and referrals based on participant needs.
• Monitor case progress and enter information into the CARES database when necessary.
