Job summary

Services for the UnderServed in New York, NY seeks a Senior Case Manager for its homeless prevention Home Base program, located at W.181 Street. You will supervise case managers and deliver housing-related services to individuals and families in need.

Under the Program Supervisor, you will oversee intakes, assessments, service plans, referrals, and CARES database entries while coordinating supportive services.

Qualifications

• Bachelor’s Degree in Human Services field required.
• 3+ years’ experience with homeless population.
• Ability to work with a culturally diverse population.

Responsibilities

• Supervise case managers and oversee intensive case management for participants and families.
• Arrange, coordinate, monitor and deliver services related to housing needs; ensure timely intakes and assessments.
• Develop After-Care and ongoing housing assistance service planning; coordinate referrals and monitor case progress.
