Qualifications

• Two years of full-time professional experience in case management for employment and training or social services programs.

Responsibilities

• Develops, reviews and updates participant service plans and makes appropriate referrals.
• Provides intensive case management and support services for welfare recipients with employment barriers.
• Case manages and monitors participants through intake, training and job retention.
• Maintains client contacts, attendance, and progress reports.
• Acts as client advocate to the program and coordinates with partner agencies.
• Tracks data and maintains participant files and DHS system entries.
• Coordinates screenings and eligibility determinations for EARN program participation.
