The Department of Economic Security (DES), Division of Developmental Disabilities (DDD), in Avondale, AZ, is seeking a Case Manager 3 to support individuals with developmental disabilities and their families. This role emphasizes development of ISP plans, coordination of services, and community engagement to improve quality of life.

You will conduct on-site visits, collaborate with community agencies, mentor staff, and ensure timely documentation and referrals.

Formación

• Must be an Arizona licensed Registered Nurse (RN) in good standing OR possess a Bachelor Degree in Social Work (SW), OR possess a Bachelor's degree in Psychology, Special Education or Counseling AND have at least one (1) year experience providing case management services.
• For Case Managers serving elderly and/or persons with developmental disabilities with SMI, required experience is: one (1) year serving elderly and/or developmental disabilities AND two (2) years serving SMI.
• Other acceptable pathways include a Bachelor's in Human Service, Behavioral Science or related field with at least one (1) year of case management experience.

Responsabilidades

• Drive a vehicle to conduct on-site visits to members/families' homes, school or program sites.
• Develop, write, review, monitor ISP within mandated timelines and document progress and referrals.
• Research community resources to meet ISP needs and inform members of options.
• Facilitate meetings and program visits to support member/family choices; complete referrals.
• Collaborate with community agencies to provide assistance to members and families in crisis.
• Mentor and train new and existing team members in the unit.
