Job summary

The State of Arizona, Department of Economic Security, Division of Developmental Disabilities, is seeking a Case Manager 3 - Onboarding Mentor in Chandler. Qualifications

• HIPAA knowledge and privacy compliance experience.
• Familiarity with Person-Centered Service Plans.

• Ability to collaborate across agencies and vendors.

Responsibilities

• Drive a vehicle to conduct onsite visits to members' homes, schools and program sites.
• Develop and monitor Person-Centered Service Plans (PCSP) and document progress.
• Facilitate meetings and referrals for services and supports.
• Collaborate with community agencies to support members and families in crisis.
• Maintain case records, write progress notes, and attend meetings; assist supervisor as needed.
