Job summary

Qualifications

• Bachelor's degree in Social Work, Behavioral Science, or related field.
• 3–4 years progressively responsible experience providing direct case management services, including needs assessment and action planning.
• One year supervisory experience preferred, or an equivalent combination of training and experience.

Responsibilities

• Lead the case management staff and oversee an assigned caseload.
• Conduct client interviews, develop long-term program plans, and monitor progress.
• Provide comprehensive, structured case management and act as advocate to access services.
• Ensure compliance with funding requirements and maintain administrative duties.
