Job summary

The Salvation Army in Lufkin, TX is seeking a lead case management professional to coordinate services for clients in emergency shelter and transitional housing programs. The role requires a Bachelor’s degree in social work or related field and three to four years of direct case management experience, with supervisory experience preferred.

Qualifications

• Bachelor’s degree in Social Work, Behavioral Science, or related field required.
• Three to four years progressively responsible experience providing direct case management services.
• Experience developing individual recovery action plans and coordinating resources.
• One year supervisory experience preferred or equivalent.

Responsibilities

• Supervise and coordinate case management staff.
• Provide direct case management services to an assigned caseload; develop and monitor treatment plans.
