Job summary

Stony Brook Medicine seeks a TH Staff Associate/Supervising Case Manager to lead the Care Management Department, oversee Case Managers and Appeal Managers, and guide staff in discharge planning and care coordination.

The role requires NYS RN license and a bachelor’s degree, five years acute care experience, and strong leadership and communication skills; you will contribute to budget input, performance improvement, and regulatory compliance while maintaining patient-centered care.

Qualifications

• NYS RN License and a Bachelor Degree.
• Five years acute care experience.
• Experience in Case Management or Managed Care.

Responsibilities

• Provide clinical services to patient population and oversee staff including Case and Appeal Managers.
• Input into department budget, utilization review, staffing and systems and processes.
• Mentor staff and coordinate department orientation of new staff.
• Ensure discharge planning and care coordination align with regulatory requirements.
• Maintain high level of customer service and support interdisciplinary teams.
