Stony Brook Medicine is seeking a TH Staff Associate/Case Manager to provide clinical services in the Care Management Department. You will support inpatient and observation cases, review utilization, and collaborate with the team to ensure appropriate level of care.

Qualifications include a Bachelor\'s degree or RN license with hospital experience, strong communication and documentation skills, and readiness to participate in performance improvement and discharge planning.

Qualifications

• Bachelor’s degree or nursing degree with RN license and 5 years hospital experience.
• Ability to document accurately and prioritize cases.
• Strong interpersonal and communication skills.

Responsibilities

• Reviews utilization for inpatient and observation cases and discusses plan of care with the team.
• Reviews discharge planning and coordinates with interdisciplinary team for safe discharge.
• Communicates with physicians, payers, and transferring hospitals for authorization.
• Maintains EMR documentation of acuity, plan of care and communications.
• Participates in performance improvement activities.
