ACMH, Inc. in The Bronx is seeking a Senior Case Manager/Case Manager for the Bronx Treatment Apartment (BTA).
The role provides case coordination and delivers community rehabilitation services, reporting to the Program Director, on a 9:00am-5:00pm, Monday–Friday schedule. Responsibilities include home visits, psychosocial assessments, coordinating psychiatric and housing services, developing individualized plans, crisis intervention, and thorough documentation.

Sr. Case Manager Qualifications: M.A. in psychology or a human service related field. B.A. in psychology or a human service related field with a minimum of 3 years of experience providing direct care in human services. Experience working with adults with serious mental illness preferred. Strong written and verbal communication skills. Master’s Degree preferred.
Scattered Site SCM Salary: $63,905 plus generous benefits

Scattered Site CM Salary: $55,632 plus generous benefits
