Inova Fairfax Hospital in Falls Church, VA is seeking an Experienced Case Management Program Manager to join the Case Management Team. This full-time, day-shift role runs Monday to Friday 8:30 am to 5:00 pm with rotating weekends and holiday rotation.

Qualifications

• BLS certification required at start; ACM or CCM certification preferred on start.
• Master's Degree in Social Work or RN license in Virginia required.
• At least 5 years of acute care case management experience.
• Education: BSN or MSW.

Responsabilités

• Guides discharge planners and the multidisciplinary team through training and consultation.
• Ensures continuity of care and appropriate referrals for Social Determinants of Health (SDOH) needs.
• Initiates referrals to specialists, home health, hospice, SNF and other services as indicated.
• Communicates status of care plans with patients, families, and care teams; resolves issues impeding progress.
• Seeks consultation from relevant disciplines to expedite care and facilitate discharge; coordinates with payers as needed.
