Job summary

Hospital Sisters Health System (HSHS) seeks a Manager-Case Management in Springfield, IL to lead the Case Management team and drive care coordination across stakeholders. The role focuses on assessment, planning, facilitation, and advocacy to meet patients' comprehensive health needs.

Required are a bachelor's in Nursing or Social Work with at least 3 years in leadership and 5 years in care management, licensed in IL or WI, with CCM/ACM preferred.

Qualifications

• Bachelor's degree in Nursing or Social Work is required.
• Master's degree in Nursing or Social Work is preferred.
• Licensure as RN or Social Worker in IL or WI is required.
• 3 years of leadership in a progressively responsible role is required.
• 5 years of care management, home care, social work, or relevant experience is required.
• Case Management certification (ACM CCM RN-BC) is preferred.
