Job summary

Hospital Sisters Health System (HSHS) is seeking a Manager-Case Management to join our team at St. John's Hospital in Springfield, IL.

This full-time salaried leadership role oversees assessment planning, care coordination, evaluation, and advocacy to meet patients’ comprehensive health needs. Requirements include a Bachelor's in Nursing or Social Work (Master's preferred); 3 years of leadership experience and 5 years in care management or related field.

Qualifications

• 3 years leadership in a progressively responsible role.
• 5 years care management, home care, social work, or relevant experience.
• Licensed Registered Nurse or Social Worker in IL or WI.
• Case Management Certification (ACM CCM RN-BC) preferred.

Responsibilities

• Lead and supervise the case management team across stakeholders.
• Oversee assessment planning, facilitation, and care coordination.
• Evaluate and advocate for options and services to meet patient needs.
• Collaborate with hospital departments across Illinois and Wisconsin.
