Qualifications

• 2 years of therapy experience (individual/family/group).
• Knowledge of addiction disorders, stigma, recovery and relapse.
• Excellent interpersonal skills.
• Strong knowledge of available community resources.

Responsibilities

• Performs concurrent patient assessment to determine level of care and expedite the plan of care.
• Provides consultations to medical staff on socio-emotional factors affecting illness and plan of care.
• Documents discharge planning services to facilitate post-hospital care needs.
• Collaborates with interdisciplinary team to transition patient to next level of care.
• Uses evidence-based standards to guide care evaluation, length of stay, and medical necessity; consults RN Case Manager for complex cases.
• Intervenes for patients with needs in abuse, neglect, mental health, end of life, ethics, legal, finances, and family dynamics.
• Facilitates guardianship, adoption, advance directives, and emergency detentions as needed.
• Develops relationships with community organizations to support in-hospital and post-discharge care.
