Job summary

The Case Management Coordinator at HCA HealthONE Swedish will oversee coordination of services through an interdisciplinary process that provides a clinical, financial and psychosocial approach across the continuum of care.

Discharge planning will begin early and continue throughout hospitalization. This role collaborates with physicians, social workers, and administration to improve patient outcomes and manage resources efficiently.

Qualifications

• Current Colorado RN licensure or active multistate license.
• BLS certification required.
• UM/CM certification preferred.
• Graduate of accredited nursing or social work program.
• RN with BSN or MSW preferred for social work.

Responsibilities

• Coordinate discharge planning with the care team.
• Identify barriers to discharge and report to administration.
• Collaborate with Director to evaluate staff and develop action plans.
• Lead staff orientation and annual core competency evaluations.
