Job summary

The Clinical Coord Case Manager at HCA HealthONE Swedish supports care coordination and delivery of quality patient care within the hospital's care team. You will work with discharge planners, social workers, UR RNs, and physicians to determine the plan of care and ensure timely post‑discharge arrangements.

Minimum Colorado RN licensure with BLS, 3–5 years acute care experience and 2+ years supervisory experience are preferred.

Qualifications

• Current RN license in Colorado or multistate license; BLS certification required.
• BSN degree preferred; 3–5 years acute care experience; 2+ years supervisory experience.
• Excellent communication, critical thinking, and coordination with care team.
• Ability to assess information, define outcomes, and report metrics.

Responsibilities

• Deliver care coordination and high‑quality patient care.
• Collaborate with discharge planners, social work, UR RN to determine plan of care.
• Communicate with post‑acute vendors to coordinate referrals.
• Report length‑of‑stay issues and barriers to discharge to management.
