Job summary

Life Care Center of Evergreen, located just west of Lakewood, is seeking a PRN Speech Language Pathologist to join our rehab team. The SLP provides direct speech therapy per physician referral in a 5-star, compassionate post-acute setting.

Applicants should have a MS/MA in SLP, ASHA CCCs or be eligible for CFY, active state licensure, and CPR certification. New grads are welcome; CPR must remain current throughout employment.

Qualifications

• Graduate of accredited Speech-Language Pathology program (MS/MA).
• ASHA CCCs or eligible for CFY to earn CCCs and maintain credentials.
• Current state licensure in good standing throughout employment.
• One year experience preferred; new grads welcome to apply.
• CPR certification upon hire or during orientation and kept current.

Responsibilities

• Provides direct rehab care in speech therapy per physician referral.
• Liaises with patients, families, and departments to plan patient needs.
• Maintains confidentiality and follows Life Care policies and procedures.
• Charts appropriately and timely; utilizes therapy software.
• Demonstrates excellent customer service and interdisciplinary teamwork.
