The role involves assessments, care planning, discharge coordination, and advocating for residents’ rights while maintaining compliant documentation and contributing to a supportive, growth-oriented team environment.

Qualifications

• Bachelor’s or Master’s degree in Social Work (BSW or MSW).
• Prior experience in long-term care or healthcare setting preferred.
• Strong interpersonal, organizational, and problem-solving skills.
• Knowledge of federal and state regulations related to nursing facilities.
• Empathetic, professional, and resident-centered approach.

Responsibilities

• Assist residents and families in adjusting to short-term rehab or long-term care placement.
• Conduct psychosocial assessments and care planning.
• Coordinate discharge planning and referrals.
• Advocate for residents’ rights and help resolve concerns.
• Participate in care plan meetings and interdisciplinary team discussions.
• Maintain accurate documentation in compliance with state and federal regulations.
