Position: Social Worker
Location: Tinton Falls, NJ 07724
Responsibilities:
Provide psychosocial support and counseling to residents and their families
Complete social service assessments and maintain accurate documentation
Develop and implement individualized care plans
Assist residents and families with adjustment to the facility and changes in health or circumstances
Participate in care plan meetings and interdisciplinary team discussions
Advocate for residents' rights, needs, and preferences
Coordinate discharge planning and appropriate community resources
Communicate with families, healthcare providers, and outside agencies
Address resident concerns and help resolve social and emotional issues
Provide support during transitions of care
Maintain confidentiality and comply with facility policies and applicable regulations
Qualifications:
Bachelor's degree in Social Work or related field required
MSW preferred
LSW/LCSW preferred or required based on applicable regulations
Previous experience in skilled nursing, long‑term care, healthcare, or geriatric services preferred
Excellent communication and interpersonal skills
Compassionate, patient, and professional approach
Strong organizational and documentation skills
Ability to work effectively as part of an interdisciplinary team
