Qualifications

• Bachelor's degree in Social Work or related field required.
• MSW preferred.
• LSW/LCSW preferred or required based on applicable regulations.

• Strong organizational and documentation skills.
• Ability to work effectively as part of an interdisciplinary team.

Responsibilities

• Provide psychosocial support and counseling to residents and their families.
• Complete social service assessments and maintain accurate documentation.
• Develop and implement individualized care plans.
• Assist residents and families with adjustment to the facility and changes in health or circumstances.
• Participate in care plan meetings and interdisciplinary team discussions.
• Advocate for residents' rights, needs, and preferences.
• Coordinate discharge planning and appropriate community resources.
• Communicate with families, healthcare providers, and outside agencies.
• Address resident concerns and help resolve social and emotional issues.
• Provide support during transitions of care.
• Maintain confidentiality and comply with facility policies and applicable regulations.
