Job summary

Pleasant Acres Nursing & Rehabilitation Center is seeking a dedicated Social Worker in York, PA to support residents’ emotional and social well-being. You will collaborate with the interdisciplinary team to develop care plans, provide counseling, and facilitate communication with families and healthcare providers.

Responsibilities include assessments, care planning, documentation, and active participation in quality improvement to enhance resident care and outcomes.

Qualifications

• Degree in Social Work or related field required, Bachelor's preferred.
• 2 years of experience in social work in a long-term care environment.
• Experience with PointClickCare or other health information management system; proficiency using Microsoft Word, Excel and Outlook.
• Excellent interpersonal and listening skills, patient/resident focused, the proven ability to set priorities, problem solve and manage conflict.

Responsibilities

• Assess the emotional, social, and psychological needs of residents and develop individualized care plans to meet those needs.
• Provide counseling and support services to residents and their families, addressing issues related to adjustment, coping, and loss.
• Facilitate communication between residents, families, and healthcare providers to ensure that residents' needs are met and concerns are addressed.
• Coordinate with other members of the healthcare team to ensure that residents receive comprehensive and coordinated care.
• Develop and implement strategies to promote socialization and community building among residents.
• Maintain accurate and up-to-date records of resident assessments, care plans, and progress.
• Participate in quality improvement initiatives to improve the overall quality of care and services.
