Job summary

Pleasant Acres Nursing & Rehabilitation Center in York, PA is seeking a dedicated Social Worker to support residents' emotional and social well-being, collaborate with an interdisciplinary team, and create individualized care plans.

You will provide counseling to residents and families, coordinate with health providers, document assessments, and contribute to quality improvement. A Bachelor's in Social Work and at least two years in long-term care are preferred.

Qualifications

• Degree in Social Work or related field required, Bachelor's preferred.
• 2 years of experience in social work in a long term care environment.
• Experience using PointClickCare or other health information management system; proficiency using Microsoft Word, Excel and Outlook.
• Excellent interpersonal and listening skills, patient/resident focused, the proven ability to set priorities, problem solve and manage conflict.

Responsibilities

• Assess the emotional, social, and psychological needs of residents and develop individualized care plans to meet those needs.
• Provide counseling and support services to residents and their families, addressing issues related to adjustment, coping, and loss.
• Facilitate communication between residents, families, and healthcare providers to ensure that residents' needs are met and concerns are addressed.
• Coordinate with other members of the healthcare team to ensure that residents receive comprehensive and coordinated care.
• Develop and implement strategies to promote socialization and community building among residents.
• Maintain accurate and up-to-date records of resident assessments, care plans, and progress.
• Participate in quality improvement initiatives to improve the overall quality of care and services.

We are seeking a dedicated and compassionate Social Worker to join our team. As a Social Worker, you will play a vital role in ensuring the emotional and social well-being of our residents.
