Job summary

Qualifications

• LSW license is required.
• Experience in skilled nursing or long-term care preferred.
• Knowledge of elder care best practices.

Responsibilities

• Assess and address social and emotional needs of residents.
• Develop and manage individualized care and discharge plans.
• Serve as liaison among residents, families, medical staff, and external agencies.
• Maintain documentation including MDS 3.0 and CAA’s.
• Advocate for residents’ rights and ethical care practices.
• Coordinate psychological services and hospice referrals as needed.
• Participate in interdisciplinary care planning and quality improvement efforts.
• Ensure compliance with state and federal regulations regarding social services.
