Heritage Skilled Care is seeking a Licensed Social Worker (LSW) to deliver professional social work services within a long-term care facility. The LSW will support residents’ emotional and psychosocial needs, promote rights and quality of life, and ensure compliance with CMS and state regulations.

The role involves psychosocial assessments, care planning, resident and family counseling, discharge planning, resource coordination, and advocacy.

Formación

• BSW required.
• Current state licensure as an LSW in good standing.
• 1-2 years of healthcare or long-term care experience preferred.

Responsabilidades

• Perform psychosocial assessments and develop care plans.
• Provide counseling and emotional support to residents and families.
• Assist with discharge planning and transitions to other care settings.
• Advocate for residents' rights and ensure regulatory compliance.
• Document assessments, interventions, and progress notes per standards.
