Job summary

Psycho Social Therapies is seeking an LSW/LISW or LPC/LPCC to provide licensed social work or counseling services in nursing homes and assisted living facilities across Ohio. You will evaluate clients, create care plans, collaborate with care teams, and deliver talk therapy.

This role supports geriatric residents facing anxiety, depression, and related mental health challenges. Onboarding, benefits, PTO, flexible scheduling, and an upbeat work environment are offered.

Qualifications

• Must hold LSW/LISW or LPC/LPCC licensure in Ohio.
• Experience with geriatric populations in nursing home settings preferred.
• Strong assessment and individualized care planning skills.
• Ability to coordinate with healthcare teams and case managers.

Responsibilities

• Evaluate clients to understand needs and mental health conditions.
• Develop individualized care plans for each client.
• Collaborate with healthcare professionals to coordinate services.
• Monitor progress and adjust plans as needed.
• Maintain accurate and confidential records of interventions.
• Provide talk therapy addressing emotional and behavioral health.
