Job summary

The Alden Network seeks a Social Service Director to lead the Social Service Department within the facility, ensuring compliance with federal, state and local LTC standards. You will manage assessments, care planning, and resident advocacy while coordinating with families, staff, and community agencies.

You will supervise social service staff, participate in quality initiatives, and drive effective discharge planning and IDT collaboration to enhance resident well-being.

Qualifications

• Bachelor’s Degree in a Human Service field; licensure in human services preferred.
• One year experience in the social service realm of healthcare.
• Knowledge of Federal and State LTC Social Service regulations and interpretation.
• Ability to engage with residents, families, staff and community agencies.
• Strong reading/writing/speaking English; capable of concise clinical reports.
• Independent decision-making with professional discretion.
• Ability to lead and work harmoniously with others.
• Willingness to accept supervision and in-service training.
• Confidential data handling and professional ethics.
• Marketing the Alden’s Social Services Program to the community and professionals.
• Desire to work with diverse populations in long/short-term care.

Responsibilities

• Welcome new admissions and establish a trusting therapeutic relationship.
• Assess and document residents’ feelings about LTC placement and changes.
• Develop person-centered social service plans based on detailed resident data.
• Document interactions and advocate for residents per state standards.
• Plan care with IDT and contribute to the resident care plan.
• Contribute to discharge planning with interdisciplinary input.
• Schedule supportive visits and write progress notes on changes.
• Provide counseling and behavioral programming support to residents.
• Act as liaison between residents, families, facility, and community agencies.
• Maintain records, participate in quality improvement and safety committees.
• Supervise, educate, counsel social service staff; provide ongoing assessments.
• Complete initial/social service assessments, MDS/CAAs, and care planning.
• Enter initial assessments within 48–72 hours and re-evaluate periodically.
• Address behavioral incidents with proactive documentation and follow-up.
• Perform related duties as assigned.
