Job summary

Normandy Nursing Center is seeking a Social Services Assistant to integrate discharge planning within the hospital system and coordinate care with residents, families, and care teams. You will develop realistic plans, assess psychosocial needs, and promote residents' rights while ensuring smooth admissions.

Qualifications

• Bachelor's degree in social work preferred.
• Current state clinical social work licensure required.
• Short term rehab experience preferred.
• Two years of Social Services background or a behavioral setting preferred.
• Able to communicate effectively in English, both verbally and in writing.
• Bilingual preferred.

Responsibilities

• Assures that social service/discharge planning activities are integrated and coordinated within the hospital system by effective communication at all levels.
• Develops specific, measurable, realistic plans and objectives to enhance social service delivery.
• Performs psychosocial assessment and reassessment on all residents.
• Interacts with residents and families to form the plan of care.
• Coordinates Social Service/Discharge Planning to meet resident needs and facility policy.
