Maryville is a faith-based nonprofit care facility in Beaverton, Oregon, seeking a full-time Social Services Coordinator to join our team of health-care professionals dedicated to compassionate senior care.

The role focuses on assessing residents' psychosocial needs, developing care plans, coordinating with families and community agencies, and ensuring HIPAA-compliant confidentiality. Weekend work may occasionally be required.

Qualifications

• Bachelor's degree in Social Work or related healthcare field.
• Ability to pass a background check.
• Recent experience in a Social Services role in a nursing home/hospital setting or similar.
• Ability to work in a fast-paced environment.
• Excellent communication and customer service skills.
• Strong computer skills for EMR environment.

Responsibilities

• Develop assessments and written plans of care for residents.
• Assess residents' psychosocial and emotional needs.
• Communicate residents' condition with other departments and agencies.
• Ensure social and emotional needs of residents are met.
• Participate in care planning conferences.
• Assist in discharge planning and inform families of programs.
• Serve as liaison with community agencies to secure services.
• Make referrals and document outcomes.
• Maintain confidentiality per HIPAA guidelines.
