Job summary

Maryville seeks an experienced Social Services Coordinator in Beaverton, OR for a full-time role serving seniors with dignity and compassion.

You will develop assessments and care plans, coordinate psychosocial support, and liaise with departments and community agencies to ensure needed services are provided promptly.

Qualifications

• Bachelor’s degree is required; focused in Social Work or other Healthcare related fields required.
• The ability to pass a background check is required of all Maryville Employees.
• Recent experience in a Social Services role in a nursing home/hospital setting or some other Social Worker role required.
• Ability to work in a fast-paced work environment.
• Excellent communication and customer service skills.
• Strong computer skills due to EMR environment.

Responsibilities

• Developing assessments and written plans of care to identify the needs of each resident.
• Assessing the resident's psychosocial and emotional needs.
• Effectively communicating information concerning a resident’s condition with other departments and government agencies.
• Ensuring the medically related emotional and social needs of the resident are met and maintained.
• Participating in resident care planning conferences and discharge planning.
• Serving as a liaison with community agencies to coordinate ancillary services within requested timeframes.
• Making referrals and documenting outcomes.
• Maintain confidentiality of all resident care information per HIPAA guidelines.

Physical requirements:

• Must be able to lift 25 pounds at times.
• Required to stand or walk for prolonged periods.
• Ability to lift, push, pull or move at least 50 pounds independently.
• Requires regular bending, stooping, reaching, and kneeling.
