Job summary

Hemet Hills Post Acute in Hemet, CA is seeking a compassionate Social Services Assistant to join our interdisciplinary team. You will coordinate services, facilitate communication with residents and families, assist with discharge planning, and contribute to a positive resident experience.

The ideal candidate has a high school diploma and experience in healthcare or social services; an associate's or bachelor's degree is preferred.

Qualifications

• High school diploma or equivalent required.
• Associate's or Bachelor's degree in Social Work, Human Services, Psychology, or a related field preferred.
• Previous healthcare, skilled nursing, rehabilitation, or social services setting preferred.
• Strong interpersonal, communication, and organizational skills.
• Ability to work effectively with residents, families, and healthcare professionals.
• Proficient with basic computer applications and electronic health records preferred.
• Ability to maintain professionalism, empathy, and confidentiality in all interactions.

Responsibilities

• Assist the Social Services Director with resident assessments, care planning, and discharge planning activities.
• Support residents and families by providing information regarding available resources and community services.
• Maintain accurate and timely documentation in accordance with facility policies and regulatory requirements.
• Coordinate appointments, transportation, and referrals as needed.
• Help address resident concerns and support resident rights and quality of life initiatives.
• Participate in interdisciplinary team meetings and care conferences.
• Assist with admissions, transfers, and discharge processes.
• Promote a positive and supportive environment for residents, families, and staff.
• Maintain confidentiality and comply with HIPAA regulations.
