Job summary

Abington Manor is seeking a full-time Social Service Coordinator with a Bachelor’s degree in Social Work or related field and experience serving the elderly in long-term care. The role involves assisting the Director of Social Services, assessing residents’ psychosocial needs, and coordinating access to services to improve quality of life.

The position requires strong communication and social work skills, the ability to collaborate across disciplines, and adherence to state/federal regulations.

Qualifications

• Must have a Bachelor's Degree in Social Work or related field.
• Minimum three years supervised social work experience in a health care setting with elderly preferred.
• Experience with elders in community or long-term care setting preferred.
• Excellent communication, assessment and social work skills.
• Ability to work with multiple disciplines and understand state/federal regulations.

Responsibilities

• Assist to the Director of Social Services.
• Address residents' psychosocial, mental and emotional needs and aid in accessing services.
• Exercise professional judgment to maximize residents' well-being and quality of life.
• Perform administrative duties, advocacy, clinical assessment and care planning, and interventions.
• Establish referral sources and admit residents efficiently according to policy.
