Job summary

Abington Manor is seeking a Social Service Coordinator (Full Time) to support nursing home residents in identifying psychosocial and emotional needs and coordinating access to services.

Applicants must have a Bachelor's Degree in Social Work or related field and at least three years of supervised social work experience in a health-care setting; elder-care experience is preferred. Daily pay, comprehensive benefits, 401k, PTO, and more are offered.

Qualifications

• Bachelor's Degree in Social Work or related field required.
• Minimum three years supervised social work experience in a health care setting.
• Prior work with elders in a community or long-term care setting preferred.
• Must demonstrate communication, assessment and social work methods/techniques.
• Ability to work with diverse disciplines and understand state/federal regulations.
• Post offer, pre-employment background check and drug screen required.

Responsibilities

• Assist the Director of Social Services.
• Identify residents' psychosocial, mental and emotional needs and facilitate access to services.
• Use professional judgment to maximize residents' well-being and quality of life.
• Conduct administrative duties, advocacy, residents' rights, and care planning.
• Establish referrals and admit residents in accordance with policies and procedures.
• Perform additional duties related to the position.
• Conduct tours and initiate admission processes.
