Job summary

PACE Southeast Michigan is seeking a Registered Nurse Case Manager to lead in-home nursing care for frail elders within the PACE program. You will assess needs, plan care, educate participants and families, and coordinate with a multidisciplinary team to improve functional outcomes.

Responsibilities include medication administration, monitoring responses, supervising home health aides, maintaining records under CSI policies, and participating in quality assurance activities.

Qualifications

• Must be a Registered Nurse with current Michigan licensure; BSN preferred.
• Annual mandatory in-service training including infection control, TB testing, safety training, and BLS.
• Must possess a current State of Michigan driver’s license.
• One year of experience with a frail or elderly population preferred.

Responsibilities

• Assess participants’ needs and plan care during Initial Intake and Re-Evaluation Assessments.
• Collaborate with participant, family and multidisciplinary team to develop the plan of care.
• Educate, supervise and counsel participant or caregiver on nursing care needs.
• Administer medications and treatments as ordered; monitor responses.
• Lead licensed and non-professional staff in delivering home nursing care.
• Maintain accurate and timely records and documentation per CSI policies.
