Job summary

PACE Southeast Michigan (PACE SEMI) seeks a Registered Nurse Case Manager to coordinate comprehensive care for frail elders in the PACE program, conducting assessments, planning, implementing, and evaluating care to promote autonomy.

The RNCM leads licensed and non-professional staff, delivers education, administers medications, and collaborates with the interdisciplinary team while meeting CMS competencies and delivering high-quality, patient-centered care.

Qualifications

• Must be a Registered Nurse with current Michigan licensure.
• Minimum of one year of experience with frail or elderly populations.
• Ability to lead licensed and non-professional staff and coordinate care.
• Must meet CMS-approved competency standards before independent work.

Responsibilities

• Assess participants’ needs and plan appropriate nursing care at intake and reevaluation.
• Collaborate with participant, family, and multidisciplinary team to develop plan of care.
• Provide education to participants and caregivers using adult-learning principles.
• Administer medications and treatments as prescribed and monitor response.
• Maintain accurate records and report changes to medical personnel.
