Job summary

Medix seeks a Referral Coordinator to support a senior patient population by coordinating referrals and helping members access specialty care. The role focuses on scheduling appointments, patient outreach, and documenting referrals in the EMR.

The ideal candidate is organized, detail-oriented, phone-focused, and able to manage multiple referrals and patient needs simultaneously, while maintaining HIPAA confidentiality.

Qualifications

• High school diploma or equivalent.
• 2+ years in referral management or healthcare coordination.
• Experience scheduling referrals and using EMR systems.
• Strong communication and organizational skills.
• Phone-based role experience.

Responsibilities

• Coordinate referrals from primary care to specialists and external providers.
• Schedule specialist appointments for patients across multiple specialties.
• Conduct phone outreach about upcoming appointments and screenings.
• Follow up with patients to reschedule or complete missed appointments.
• Coordinate with outside provider offices for referrals, records, and follow-up.
• Serve as point of contact for patients and families regarding referrals.
• Document information from outside providers and maintain HIPAA confidentiality.
