Senior CommUnity Care of Northern KY PACE is seeking a Physical Therapy Assistant to deliver clinical PT services under the supervision of a licensed therapist. The role focuses on acute, restorative and maintenance needs of older adults in our PACE program, with emphasis on home and community settings.

Responsibilities include implementing treatment plans, documenting progress, and communicating changes to the supervising therapist.

Qualificações

• Must have PTA license in Kentucky.
• Graduate of a PTA curriculum with active Kentucky PTA license.
• Minimum of one year experience with frail or elderly population.
• Must clear background check and immunizations.
• Must have valid driver's license and transportation to home visits.

Responsabilidades

• Provide PT services under supervision for acute, restorative and maintenance needs.
• Follow treatment plans and communicate progress to supervising PT.
• Maintain documentation according to state policies and CMS requirements.
• Maintain records in the EMR system per policy.
• Perform other duties as assigned.
