Qualifications

• MD or DO degree from an accredited medical school.
• Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatrics.
• Experience in Managed Care, Value-Based Care, or Medicare Advantage preferred.

Responsibilities

• Provide comprehensive primary care services to senior patients age 65+.
• Develop individualized care plans and manage chronic conditions.
