Qualifications

• MD/DO degree from an accredited program.
• Current and unrestricted medical license in state of practice; eligible for other states.
• Active and unrestricted DEA license; Medicaid provider number.
• Board certification or eligibility in Family Medicine, Internal Medicine or Geriatric Medicine preferred.
• Strong interpersonal skills and a collaborative, team-based approach.
• Proficiency with basic computer tools and electronic medical records (EMR).
• Willingness to be screened for TB as part of employment.
• Experience in value-based care settings is a plus.

Responsibilities

• Evaluates and treats center patients in accordance with standards of care.
• Coordinate patient services including referrals, hospital and SNF coordination, and home health care.
• Act as a key medical resource within the care team during daily huddles.
• Lead Physician and Center Administrator with professional, approachable demeanor.
• Complete medical records daily with diagnosis and coding support for quality data.
• Follow policies defined by Clinical Leadership and review outcomes with leadership.
• Direct patient care time balanced with administrative tasks.
