Legacy Healthcare Services, Inc. is seeking a dedicated Physical Therapist to join our team providing resident-centered rehabilitation. You will plan and implement personalized treatment programs, work closely with a multi-disciplinary team, and participate in clinical conferences to ensure optimal patient outcomes.

Qualifications

• Degree in area of discipline, and current PT licensure in state of practice or ability to obtain prior to start.
• Strong interpersonal, clinical, and numerical skills to support patient care and documentation.
• Proficiency with common computer software and electronic medical records.

Responsibilities

• Provide rehabilitative services to residents under medical orders as part of a coordinated team.
• Plan initial and subsequent treatment programs based on clinical findings.
• Treat residents one-on-one with individualized, function-based sessions.
• Participate in clinical conferences to coordinate plans of care and treatment goals.
