Qualifications

• Advanced degree in Physical Therapy from an accredited program.
• Current license to practice Physical Therapy in the state of Illinois.
• Valid driver's license and auto insurance proof.
• Two years on an interdisciplinary team in hospital, nursing home, or community-based setting preferred.
• Experience with frail or elderly populations preferred; training provided if not present.

Responsibilities

• Provide high-quality PT care as a member of the IDT for PACE participants.
• Perform initial and ongoing in-person assessments and develop treatment plans.
• Assess home safety and recommend equipment or modifications.
• Educate participants and families on procedures and home programs.
• Document care accurately and timely in medical records.
