Job summary

BoldAge PACE is seeking a Physical Therapist in Crestwood, IL to plan, organize, and implement PT services for program participants. Responsibilities include assessment, treatment, and teaching participants, caregivers, or families to maintain home and community support.

As a member of the interdisciplinary team, you will conduct initial and ongoing assessments, recommend equipment and home modifications, and document progress while ensuring adherence to standards and policies.

Qualifications

• Advanced degree in Physical Therapy from an accredited school.
• Current license to practice Physical Therapy in the state.
• Two years of experience on an interdisciplinary team preferred.
• 1 year of experience with frail or elderly population preferred. Training provided if needed.

Responsibilities

• Provides high quality care as a member of the IDT.
• Performs in-person assessments at enrollment and ongoing as determined by the IDT.
• Plans physical therapy treatment programs according to recognized standards and policies.
• Documents evaluation, assessment, plan and treatment rendered accurately and timely.
• Educates participants/family in procedures, home programs and documentation of training.
• Collaborates with the IDT to develop a comprehensive care plan for each participant.
