The role offers dedicated administrative time, in-house consultants in cardiology and podiatry, and access to holistic health services. A pathway to partnership exists with growth opportunities.

Qualifications

• MD or DO degree from an accredited institution.
• Board certified or board eligible in Family Medicine, Internal Medicine, or Geriatric Medicine.
• Valid medical license to practice.
• Experience in value-based care preferred.
• Experience caring for senior patients with chronic conditions is beneficial.

Responsibilities

• Provide comprehensive primary care to a senior patient population with multiple chronic conditions.
• Maintain a small patient panel to ensure personalized and attentive care.
• Collaborate with a dedicated care team, including MA, scheduler, and referral specialist.
• Utilize in-house consulting specialists and holistic health services to support integrative care.
• Engage in value-based care initiatives to improve patient outcomes.
• Participate in administrative activities with time allocated in schedule.
• Work Monday through Friday, with clinic hours from 7:30 am to 5:00 pm.
• No required nights or weekends; 100% outpatient care.
