gobravvo is seeking a Primary Care Physician (PCP) to lead senior-focused, value‑based care in Cleveland, Ohio. This full‑time role emphasizes outpatient, team‑based care with a physician‑led culture, focusing on quality outcomes and patient experience.

Qualifications

• Residency training completed in Internal Medicine or Family Medicine.
• Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatric Medicine.
• Commitment to value-based, patient-centered care and continuous professional development.
• Strong interpersonal, communication, and organizational skills.
• Ability to work collaboratively in a multidisciplinary team.

Responsibilities

• Deliver comprehensive primary care to a senior patient panel of up to 450 individuals with 3–5 chronic diseases.
• Conduct 18–22 patient visits per day in a 100% outpatient setting.
• Collaborate with a dedicated care team to provide seamless patient experiences.
• Maintain a work schedule Monday–Friday, 7:30 a.m.–5:00 p.m. (last patient 4:15 p.m.).
• Utilize administrative time for thorough documentation and care planning.
• Engage in value‑based care initiatives to improve outcomes and support organizational goals.
• Coordinate with in‑house specialists (cardiology, podiatry, etc.) for holistic care.
• Provide patient education and preventive care tailored to seniors.
