Job summary

Oasis Therapy Services in Green Bay, Wisconsin seeks a Physical Therapist (PT) to deliver one-on-one rehabilitation in an assisted living and memory care setting. You will evaluate mobility and gait, develop individualized plans, and work with residents and caregivers to promote safe independence.

The role offers PRN employment with Net Health EMR, flexible scheduling, and competitive compensation. Active Wisconsin PT license and CPR are required; geriatric experience is preferred and

• Current CPR certification.

Responsibilities

• Evaluate mobility, strength, balance, gait, and fall risk.
• Develop and provide individualized treatment plans to improve safe mobility and independence.
• Monitor progress and adjust treatment plans as needed.
• Educate residents and caregivers on exercises, transfers, fall prevention, and assistive devices.
• Complete accurate Net Health documentation and collaborate with the care team.
